Job Description

The Hepatitis C Project Coordinator is responsible for the overall planning, coordination, and implementation of community-based initiatives for this project.  S/he will collaborate with the joint Sioux Lookout First Nations Health Authority (SLFNHA), Toronto Centre for Liver Disease (TCLD), and Public Health Agency of Canada (PHAC) team to undertake planning processes with community leaders and health system providers, coordinate training of community-based workers, and oversee quality control and monitoring and evaluation data.   

•University degree or college diploma in a health related field;
•Previous experience in health planning and project management;
•Previous experience in health promotion and education;
•Previous experience in blood borne infections, harm reduction and/or public health
•Understanding of the unique determinants of health for First Nations in northwestern Ontario, and the health landscape in which they seek services;
•Active interest in hepatitis C and harm reduction as it pertains to individuals who use substances;
•Excellent organization, time management, problem-solving, strategic planning, facilitation and decision-making skills;
•Experience with monitoring and evaluation, community-based research and knowledge translation methodologies;
•Professional integrity and the ability to maintain confidentiality is essential;
•Experience working both independently and in a team-orientated, collaborative environment is essential;
•Ability to develop and maintain respectful/cooperative relationships with First Nations people, communities, service stakeholders, and health care providers to promote an integrated seamless delivery of service;
•Excellent written, verbal, and computer skills;
•Ability to complete reports and documentation;
•Ability to communicate in one of the First Nations dialects within the Sioux Lookout Zone an asset;
•Must be willing to travel.

1.   Provide overall coordination of the Sioux Lookout and community-based components of the initiative;
2.   Conduct community consultation processes such as community visits, meetings, video conferences and teleconferences;
3.   Develop detailed work-plans with the project team, community leaders and local health providers;
4.   Coordinate training of community-based workers to support increased access to testing for blood-borne infections;
5.   Provide logistical support for community-based testing initiatives;
6.   Coordinate data collection for ongoing monitoring and evaluation of the project;
7.   Collaborate with existing SLFNHA initiatives for harm reduction and control of blood born infections and contribute to broader SLFNHA program planning processes as requested;
8.   Write reports and proposals, as necessary, to support the development of the project;
9.   Contribute to the development of methods and tools for sharing and disseminating knowledge generated by the project;
10. Act as a liaison and represent the project as requested by the project team.
